IGNITE PERFORMANCE ACADEMY
ATHLETE PARTICIPATION WAIVER & RELEASE
I understand that participation in IGNITE Performance Academy activities operated by Stafford Operations LLC, doing business as Stafford Fitness/Wellness and IGNITE Performance Academy, involves physical activity and athletic training, including but not limited to sprinting, jumping, agility drills, strength and conditioning exercises, athletic combine testing, reaction drills, group training activities, and sports performance exercises. Activities may take place indoors and outdoors. I understand that participation in these activities carries inherent risks including, but not limited to, slips, falls, muscle strains, sprains, joint injuries, heat-related illness, physical injury, serious injury, or death. I voluntarily allow my child to participate in IGNITE Performance Academy activities and assume all risks associated with participation. I certify that my child is physically capable of participating unless medical conditions or restrictions have been disclosed to staff prior to participation. I understand that Stafford Operations LLC, Stafford Wellness, IGNITE Performance Academy, and their owners, employees, coaches, trainers, contractors, volunteers, and staff are not responsible for injuries, damages, losses, or claims arising from participation, except where prohibited by law. I authorize staff to seek emergency medical treatment for my child if I cannot be reached immediately. I understand that I am financially responsible for any medical expenses incurred. I grant permission for Stafford Operations LLC, Stafford Wellness, and IGNITE Performance Academy to use photos or videos of my child for marketing, promotional, website, and social media purposes. I understand that athletes are expected to follow coach instructions and maintain safe behavior at all times. Stafford Operations LLC, Stafford Wellness, and IGNITE Performance Academy reserve the right to remove athletes for unsafe, disruptive, or inappropriate behavior without refund. By signing below, I acknowledge that I have read, understand, and voluntarily agree to the terms of this waiver on behalf of myself and my child.
